  • You must include a copy of ID and proof of ownership such as V5C, invoice or equivalent evidence.
  • Failure to include the required printed paperwork may result in refusal of service, delay, return of the item or request for additional verification.

Data Protection & Customer Declarations

UK GDPR and retention: personal data supplied through this form is processed for identity verification, ownership verification, service administration, fraud prevention, legal compliance and dispute resolution. Relevant records may be retained for up to 6 years where reasonably necessary for contractual, legal or evidential purposes.

No online document upload: this form does not request online upload of ID or ownership documents. Copies must instead be included physically in the parcel.

Customer declaration: by proceeding, the customer confirms that they are the legal owner of the vehicle, machine, module or key concerned, or that they have clear authority from the owner to request the service.

Accuracy of information: the customer confirms that all details, instructions and descriptions supplied are complete and accurate to the best of their knowledge.

Technical risk acknowledgement: the customer understands that electronic components may contain hidden faults, previous repairs, corruption, prior damage or compatibility issues that may affect outcome.

Liability position: nothing in this form excludes liability where liability cannot lawfully be excluded under applicable law. To the extent permitted by law, liability for direct proven loss arising from the service is limited to the amount paid for that service.
